Anamnez
Patient Profile
- 38-year-old male patient
- Referred to Istanbul from Russia
- Married, with expectations of an active sexual life
- No history of systemic comorbidities
- No complaints of erectile dysfunction
Reasons for presentation
- Believes his penis is too short,
- Insufficient penis girth,
- Dissatisfaction with the aesthetic appearance,
- A decrease in sexual confidence,
- Premature ejaculation
During the physical examination and measurements:
- Flaccid penis length: 8 cm
- Penile circumference: 6 cm
were recorded.
ASSESSMENT AND TREATMENT PLANNING
It was observed that the patient’s erectile function was normal. During detailed consultations, it emerged that the patient’s primary expectation was:
- a longer appearance,
- a fuller structure,
- a natural aesthetic appearance,
- increased sexual confidence
.
Following an assessment with the patient, a combined approach was planned.
Planned procedures:
- SLS (Suspensory Ligament Release)
- Penile thickening with autologous fat injection
- Hyaluronic acid (HA) application to the glans
were determined.
INTERNATIONAL PATIENT PROCESS
The patient could only speak Russian. Despite this, no communication issues were encountered during pre-operative consultations, surgical planning, or hospital organisation.

SURGICAL PROCEDURE
The operation was performed under general anaesthesia.
Procedures Performed
1. Suspensory Ligament Release (SLS)
The suspensory ligament, which anchors the penis to the pubic region, was carefully released. This procedure aimed to increase the visible portion of the penis outside the body.
2. Penile Thickening Procedure
Fat tissue harvested from the pubic region was purified through specialised processes. The prepared fat tissue was carefully injected around the penis to achieve a fuller and more natural appearance.
3. Hyaluronic Acid (HA) Application to the Penile Glans
Hyaluronic acid was applied to the glans penis. This procedure was intended to:
- an increase in volume,
- a more aesthetic appearance,
- aimed at helping to reduce complaints of premature ejaculation
was the aim.
The total duration of the procedure was approximately one hour.
POST-OPERATIVE PROCESS
The patient was monitored in hospital for 1 day following the operation.
In the early stages:
- pain control was successfully achieved,
- no circulatory problems were observed,
- no signs of infection were observed,
- the patient was mobilised comfortably.
The patient reached a clinical condition allowing them to return to their home country on the second day post-operation.
POSTOPERATIVE MEASUREMENTS
Early-stage assessment:
- Flaccid penile length: 11 cm
- Penile circumference: 9 cm
were measured.
POSTOPERATIVE RECOMMENDATIONS
To the patient:
- wound care,
- swelling management,
- hygiene guidelines,
- use of an extender,
- restrictions on sexual activity
Detailed training was provided on the subject.
The aim was to support surgical outcomes through the use of a penile extender device.
RESULT
In this case, using a combined penile aesthetic approach:
- an increase in penile length,
- a significant improvement in girth,
- a fuller appearance,
- increased patient self-confidence,
- and a supportive contribution to addressing premature ejaculation
have been targeted.
Thanks to international patient management, a multidisciplinary approach and personalised surgical planning, the process was successfully completed.
