MEDICAL HISTORY
Patient Profile:
- 59 years old
- Male
- Married, with expectations of an active sex life
- History of Type 2 diabetes (10 years)
- No history of smoking
Complaints:
- Long-standing erectile dysfunction
- Insufficient benefit from medication
- Inability to maintain an erection during sexual intercourse
- Significant reduction in quality of life and self-confidence
DIAGNOSIS
Severe Erectile Dysfunction (ED)
The patient has previously:
- PDE5 inhibitors
- Intracavernosal injections
- Vacuum device
but has not achieved a satisfactory response.
PRE-OPERATIVE PREPARATION
- Detailed urological assessment
- Blood tests and metabolic control
- Diabetes was brought under control
- The patient was given a detailed explanation of the types of prostheses (malleable / inflatable)
- A three-piece inflatable penile prosthesis was selected in consultation with the patient
- Prophylactic antibiotics were initiated to reduce the risk of infection

SURGICAL PROCEDURE
The operation was performed under general anaesthesia.
- Access was gained via a penoscrotal incision
- The cavernous structures were carefully dilated
- A prosthesis of the appropriate size was selected
- The cylinders were inserted
- The reservoir and pump system were positioned in anatomically appropriate locations
- The system was tested and its function verified
- The incisions were closed anatomically
