In the past, premature ejaculation (PE) was widely believed to be caused primarily by psychological factors. However, current scientific evidence has shown that PE is not the result of a single cause. Instead, it is now recognized as a multifactorial sexual dysfunction involving a combination of biological, neurological, hormonal, genetic, psychological, and relationship-related factors.
Research suggests that, in some men, neurochemical mechanisms involving the serotonin system, increased glans penile sensitivity, hormonal imbalances, prostate disorders, or coexisting erectile dysfunction (ED) may contribute to the development of premature ejaculation. In addition, performance anxiety, chronic stress, anxiety disorders, and relationship difficulties can significantly worsen symptoms, particularly in certain patient groups.
For this reason, successful treatment should not focus solely on prolonging ejaculation time. A thorough evaluation of the underlying causes is essential to achieve long-term and effective results. Current international urology and andrology guidelines recommend that every patient undergo an individualized assessment and that treatment be tailored according to the underlying mechanisms contributing to the condition.
How Do Psychological Factors Cause Premature Ejaculation?
Psychological factors can play an important role, particularly in men with acquired (secondary) premature ejaculation. However, it is now well established that premature ejaculation is not solely a psychological disorder.
The most common psychological factors include:
- Performance anxiety
- Negative experiences during early sexual encounters
- Chronic stress
- Depression
- Anxiety disorders
- Relationship difficulties
- Partner incompatibility
- Traumatic sexual experiences
In men with performance anxiety, fear of losing an erection or failing to satisfy a partner may unconsciously accelerate the ejaculatory reflex. Over time, this response can become a learned behavioral pattern that recurs during subsequent sexual activity.
For patients in whom psychological factors play a predominant role, medication alone may not be sufficient. Treatment plans that combine pharmacological therapy with sex therapy, couples therapy, or psychological counseling may provide better outcomes.
Biological Factors
One of the most significant advances in premature ejaculation research over the past decade has been the improved understanding of its biological mechanisms.
Although psychological causes were once considered the primary explanation, it is now recognized that biological factors play a major role in many patients.

Serotonin (5-hydroxytryptamine, 5-HT) is one of the most important neurotransmitters involved in regulating the ejaculatory reflex. Reduced serotonin activity in the brain or alterations in specific serotonin receptors may trigger ejaculation earlier than normal.

In recent years, research into premature ejaculation (PE) has expanded beyond genetics to include epigenetic mechanisms. Epigenetics refers to changes in gene expression that occur without altering the underlying DNA sequence, often as a result of environmental influences.
Researchers believe that chronic stress, lifestyle factors, hormonal changes, and environmental exposures may induce epigenetic modifications that affect the serotonin system and other neurotransmitter pathways involved in ejaculatory control.
Although current evidence is not yet sufficient to influence routine clinical practice, epigenetics is considered one of the most promising areas of future research in understanding and treating premature ejaculation.
Can Thyroid Disorders Affect Premature Ejaculation?
Hyperthyroidism is one of the hormonal disorders most strongly associated with premature ejaculation (PE). Elevated thyroid hormone levels can accelerate the body's metabolism and may also influence the ejaculatory reflex.
Clinical studies have shown that men with hyperthyroidism have a higher prevalence of premature ejaculation. In many cases, treatment of the underlying thyroid disorder has been associated with a significant improvement in ejaculatory latency.
For this reason, evaluation of thyroid function may be recommended, particularly in men who develop acquired (secondary) premature ejaculation later in life or when a hormonal disorder is suspected.
Can Prostate Diseases Cause Premature Ejaculation?
Premature ejaculation (PE) is more commonly observed in men with chronic prostatitis and chronic pelvic pain syndrome (CPPS). Although the exact mechanism has not been fully established, chronic inflammation around the prostate is thought to increase the sensitivity of nearby nerve endings and influence the ejaculatory reflex.
For this reason, evaluating for underlying prostate conditions is particularly important in men who develop acquired (secondary) premature ejaculation. Current international clinical guidelines also recommend assessing for underlying conditions such as prostatitis in patients presenting with acquired premature ejaculation, as identifying and treating the underlying cause may improve treatment outcomes.
Can Increased Glans Penile Sensitivity Cause Premature Ejaculation?
Increased glans penile sensitivity is considered one of the most important biological mechanisms associated with premature ejaculation (PE). The glans penis contains a dense network of nerve endings that detect sensations such as touch, pressure, and vibration. In some men, these nerve endings may be more numerous or more sensitive than normal. As a result, sexual stimulation may be transmitted to the brain more rapidly, triggering the ejaculatory reflex sooner than expected.
Anatomical studies have reported that men with lifelong (primary) premature ejaculation may have a greater number of branches of the dorsal penile nerve. These findings suggest that increased penile sensitivity may represent an innate biological characteristic in certain individuals.
This mechanism also helps explain why topical anesthetic creams and sprays can be effective in selected patients. By temporarily reducing the sensitivity of the glans penis, these treatments may delay the ejaculatory reflex and improve ejaculatory control.
However, increased glans penile sensitivity is not present in every man with premature ejaculation. For this reason, treatment should always be individualized based on the patient's clinical findings, underlying causes, and overall medical evaluation.
Expert Opinion
Increased glans penile sensitivity may represent an important biological mechanism, particularly in some men with lifelong (primary) premature ejaculation. However, it does not explain every case of premature ejaculation. The most appropriate approach today is to perform a comprehensive evaluation that considers psychological, biological, and neurological factors, allowing treatment to be tailored to each patient's individual needs.

Can Vitamin D and Vitamin B12 Deficiency Affect Premature Ejaculation?
In recent years, the effects of vitamin D and vitamin B12 deficiencies on male sexual health have attracted increasing scientific interest. Some studies have reported a higher risk of premature ejaculation (PE) in men with vitamin D deficiency; however, the current evidence remains inconclusive.
Similarly, although several studies have suggested an association between vitamin B12 deficiency and premature ejaculation, the underlying mechanisms have not yet been fully understood. Therefore, current international clinical guidelines do not recommend routine measurement of vitamin D or vitamin B12 levels in all patients with premature ejaculation. Nevertheless, vitamin testing may be appropriate in selected patients based on their medical history and clinical findings.
Do Obesity and Diabetes Increase the Risk of Premature Ejaculation?
Obesity and diabetes mellitus can negatively affect male sexual health by altering hormonal balance, vascular function, and the nervous system. Men with diabetes are also more likely to experience erectile dysfunction (ED) in addition to premature ejaculation.
This association cannot be explained by elevated blood glucose levels alone. Diabetes-related nerve damage, vascular changes, hormonal alterations, and psychological factors may all contribute to the development of sexual dysfunction.
Maintaining a healthy body weight, engaging in regular physical activity, and achieving good diabetes control are important not only for overall health but also for preserving sexual function.
What Is the Relationship Between Erectile Dysfunction and Premature Ejaculation?
Premature ejaculation (PE) and erectile dysfunction (ED) are not completely independent conditions. On the contrary, they frequently coexist and may negatively influence one another.
Some men with erectile dysfunction may unconsciously ejaculate sooner because of the fear of losing their erection during sexual activity. Conversely, men who experience persistent premature ejaculation may gradually develop performance anxiety, which can impair erectile quality over time.
For this reason, erectile function should always be assessed, particularly in men with acquired (secondary) premature ejaculation. The European Association of Urology (EAU) and other international clinical guidelines recommend evaluating and treating coexisting erectile dysfunction as an important part of the overall management strategy.
