Sling Procedures in Women
The most common surgical treatment and current standard treatment for stress urinary incontinence in women is midurethral sling surgery. In this procedure, a synthetic material is placed under the urethra to support its closure during activities that increase abdominal pressure (such as coughing, sneezing, bending, lifting, jumping, and running). is a treatment procedure that is performed through a small incision in the vagina and takes approximately 20 minutes. It is performed under spinal or general anaesthesia, and the patient can quickly return to their daily activities after the surgery.
In another type of female sling surgery, the pubovaginal sling, the tissue used for the sling is taken from the patient's abdominal wall or leg (fascia) or from donated tissue (cattle or cadaver).

Suspension Surgery in Men
In some men, a sling procedure may be recommended to treat stress urinary incontinence. The male sling is used to support the urethral sphincter muscle. To do this, a soft mesh band is placed under the urethra through an incision between the scrotum and the rectum. This band supports the urethra and sphincter muscle, preventing leakage by pushing the urethra upwards and allowing it to close slightly.

Cervical Suspension / Colposuspension
Burch colposuspension, or bladder neck suspension, is a surgical procedure performed to treat stress urinary incontinence in women. It involves lifting the bladder neck towards the pubic bone using permanent sutures. This is an abdominal procedure performed through the abdominal wall (muscles and skin) to access deeper pelvic areas. It can be performed as an open, laparoscopic, or robotic procedure.
Fillers (Injections)
This option is used to treat stress urinary incontinence in women by "plumping up" the inner urethral lining and narrowing the urethral opening. Modern fillers are permanent materials placed around the urethra and sphincter muscle and towards the bladder neck. This can help the natural urethral closure function to stop leakage.

Artificial Urinary Sphincter
The placement of a device called an artificial urinary sphincter (AUS) around the urethra is the most common treatment for men with stress urinary incontinence. In some cases, women may also benefit from this surgery, but due to the other surgical options mentioned earlier, it is rarely necessary in women. The AUS is a three-part device:
An artificial urinary sphincter, which is a fluid-filled cuff placed around the urethra.
A fluid-filled, pressure-sensitive balloon connected to the cuff that regulates the pressure inside the cuff. This balloon is placed in the lower abdominal region.
A pump (artificial urinary sphincter) placed in the scrotum that transfers fluid between the cuff and the balloon to open and close the cuff. The pump can be easily controlled by the patient.
During rest periods, the AUS cuff is closed to prevent leakage (it is filled with fluid). When you decide to empty your bladder, you activate the pump to push the fluid from the cuff into the balloon. This opens the urethra, allowing urine to flow and empty the bladder.
Bladder Reconstruction / Urinary Diversion Surgery
This type of major abdominal surgery is only used in very rare and complex cases.
This surgery, known as augmentation cystoplasty, is performed to enlarge the bladder in order to increase the amount of urine it can hold at one time.
These surgeries carry numerous risks and are therefore only recommended when no other options are available.
